Forms and Information

This drop-down menu provides links to common forms along with other information. If the form you need is available here, you must download and fill out the form prior to meeting with the HD Coordinator or advisor. Students who come to office hours unprepared (i.e., without the proper form completed or without required supporting documentation) will be asked to make another advising appointment once they have completed the required forms and documentation.

If you need help determining which form to fill out, or if the form you need is not available on this page, you are encouraged to meet with an HD advisor or the coordinator for guidance, but you will be expected to obtain the form and fill it out to the best of your ability before making another office hour appointment.

Course Substitutions

  • Major/Minor Course Substitution Form (ARR Correction Form): Use this form if the HD Coordinator has approved a course substitution within the HD Major.
    What must be filled out by the student before meeting with the HD Coordinator: Top portion and section labeled ‘Must be Filled Out by Student’. Leave other sections blank.
  • GE Course Substitution Form: Use this form to request a course substitution within the GE pattern.
    What must be filled out by the student before meeting with your HD faculty advisor or the HD Coordinator: Section I and Section II. Your faculty advisor or HD Coordinator will fill out and sign Section III. The student is responsible for obtaining signatures in Sections IV, V, and VI before submitting the form to Admissions and Records.

Graduation Paperwork

Getting close to being able to graduate? Congratulations!! While SSU has deadlines for graduation application submission (view them HERE), Human Development majors must apply for graduation the semester BEFORE they intend to graduate. HD majors must check their Seawolf email regularly each semester for information from the HD Coordinator regarding the deadline for filing graduation paperwork with the department. Only students who meet this deadline will be eligible to:

      • Register for HD 490 (this course now requires Consent of Coordinator and only graduating seniors will be allowed to register for it)
      • Petition to take extra units (submitting a petition early ensures students can enroll in the necessary units at the beginning of the registration period)
      • File a Major/Minor Course Substitution Form to substitute one course for another that is not available in a student's final semester (must be approved by the HD Coordinator and the student must submit a Major/Minor Course Substitution Form
  • Graduation Application: Fill out and bring a paper copy with you to your meeting with your HD advisor and the coordinator.
    What must be filled out by the student before meeting with your HD faculty advisor or the HD Coordinator: The entire application.

  • Graduation Changes: Fill out this form if you have previously submitted a Graduation Application but you need to postpone your graduation date or update the courses you need to take to complete your degree. No signatures are required.

Extra Units

  • Petition to Take Extra Units: Use this form to if you have applied to graduate and you need to enroll in more than the maximum allowed units for your final semester.
    What must be filled out by the student before meeting with the HD Coordinator: The entire form, including the information on the course you are requesting to add that will take you over the unit limit, a detailed explanation for the request, and a paper copy of your ARR, printed in color. Students who fail to attach a color copy of your ARR, printed up to 24 hours before meeting with the Coordinator, will be asked to return once they have it. Petitions for Extra Units will not be approved for students who have not filed a graduation application.

Contract Courses

Interested in engaging in an internship with a community partner or research with a faculty member? Great! The first step is to obtain a verbal agreement with the person or organization. Once you've done this, you must fill out the appropriate form, get the appropriate signatures, and submit the form by the end of Add/Drop.   • Internship Form (HD 496): Fill this form out if you are working with an approved community partner. Dr. Ben Smith is now in charge of the HD internship program. Please meet with him to discuss possible internship opportunities. The HD Coordinator will only sign off on internship forms approved by Dr. Smith.
    What must be filled out by the student before meeting with Dr. Smith: Watch the video: Filling out an Internship Agreement to see what must be filled out before meeting with Dr. Smith to get his signature. Dr. Smith will NOT sign internship forms that are not completely filled out and signed by the student and off-site sponsor.
  • Special Studies Form (HD 495): Fill this form out if you are doing independent research with a faculty member.
    What must be filled out by the student:
    The entire form. Meet with your faculty mentor for help filling out this form.

Repeat a Course

Petition to Add a Course After the Deadline

  • Petition to Add After the Deadline: Use this form if you need to add a course to your schedule after the end of Add/Drop. It is the student’s responsibility to secure all signatures. Please note the signatures required are the form must be secured from the department offering the course that is being added after the deadline. See the detailed instructions on the second page of this form.

Petition to Waive University Requirements

  • This form is available from Admissions and Records. Use this form if you want to petition to waive university-wide requirements, such as the unit requirement in a GE area. Important: Even if your advisor and/or program coordinator support your petition, the university may deny your request.
    What must be filled out by the student before meeting with an HD advisor and the HD Coordinator: The student’s information at the top of the form, and a detailed description of the university requirement you are petitioning to waive.

Petitions to Withdrawal from a Course or the University

  • Withdraw from SSU: Use this form to withdraw from SSU, either for a semester or indefinitely.
    What must be filled out by the student before meeting with an HD advisor and the HD Coordinator: The entire form, including documentation of the ‘serious and compelling reason’ for requesting to withdraw. You are responsible for understanding the potential consequences of withdrawing from the university, which can be found by reading the Withdrawal Policy.
  • Petition to Withdraw from a Course: Use this form to withdraw from a single course once the drop deadline has passed.
    What must be filled out by the student before meeting with an HD advisor and the HD Coordinator: The entire form, including the ‘serious and compelling reason’ for the course withdrawal, up to the ‘student’s signature’ line. You are responsible for understanding the potential consequences of withdrawing from a course, which can be found by reading the Withdrawal Policy.

Forms for Former SSU Students Not Currently Enrolled at SSU

  • Graduation Matriculation Form: Use this form if you previously attended SSU and wish to apply for graduation, but are not currently an enrolled SSU student. In order to apply for graduation, this form must accompany a graduation application (see Graduation Paperwork section (above) for more information about applying for graduation).
    What must be filled out by the student before submitting the form to Admissions and Records: The entire form. If you do not know the courses you still need to take to complete your degree and you no longer have SSU login credentials, please email the HD Coordinator explaining your situation. Include in your email your full name, your SSU ID number (if you know it), the semester and year you declared Human Development (if you know it), and ask the coordinator to run a ‘What-if Report’ with Human Development as your major.
  • Academic Disqualification: If you have been academically disqualified from SSU, you must follow the instructions on the Academic Disqualification webpage and bring all forms and supporting documents with you when you meet with the HD Coordinator.
