Parking Appeal Form (pdf) — 1st Level Appeal Form

Please consider filing your appeal online by visiting the Online Parking Services portal. If you prefer to fill out the physical form, please download here. This form is to be completed by registered owners or responsible parties for parking citations issued by SSU Police & Safety Services. This form must be returned within 21 days of citation issue or within 14 days of Notice of Parking Citation. Please visit our Parking Citation Appeals page for for more information.

Return this form to Parking Services by fax (707-664-2248), mail (1801 East Cotati Avenue, Rohnert Park, CA 94928) or in person at Police & Safety Services, Verdot Village.

Request for Administrative Review (pdf) — 2nd Level Appeal

This form is to be completed after notice that a citation which has been upheld at the 1st level appeal. Requests for Administrative Reviews must be received within 21 days of the notice of denied parking citation appeals.

Temporary Disability Parking Forms

Qualifying students, faculty, or staff who, due to a temporary or on-going disability, may apply for a temporary parking permit for parking in disabled zones on campus. For more information on the temporary disabled parking permits, please visit the parking permits page.

Temporary Disability Parking Application (doc)

This form is to be used by qualifying students, faculty, or staff who would like to apply for a temporary parking permit. Applications should be returned to Disability Services for Students in Salazar Hall.

Risk Management Forms

Accident Forms

Please submit all completed forms to the Risk Management office within 48-hours of the time of the incident. Risk Management will review all reports prior to submitting them to the appropriate agency. PLEASE NOTE: If you are an SSU Employee, or the Supervisor of an SSU employee, reporting a work related injury or illness, please use the forms found on the Workers' Compensation website to reoprt the incident: http://www.sonoma.edu/erc/workers-compensation/incident-reporting.html
